The following forms are in Portable Document Format (PDF format) and Adobe Acrobat Reader 7.0 or later, must be installed on your computer to access the available forms. Acrobat Reader is available free and can be downloaded here.
Those that are not familiar with Adobe Acrobat Reader, the PDF format
will enable you to read and/or print printable data files from the website preserving the exact look and content of the originals, complete with fonts and graphics.

The Indiana State USBC WBA has added a new service to the forms page. Forms indicating "fillable" allow you to
fill the form online, print it, and then mail it. You must still have the Adobe Reader mentioned above, but it makes it convenient for members to
complete the forms. In some instances, there are drop-down boxes for you to choose an option or to choose a date. Also, acquire signatures when
required.
Please remember to print
an extra copy for your records.
